How to apply
All the information for making a claim is provided on the application form. In addition you will need to provide certain details in support of your claim. The form can be completed on the screen, printed, signed and then sent to the Secretary or you can just print the form and complete it by hand. Alternatively you can email, telephone or send a fax (number on request) to the Secretary who will send you a form either electronically or by post.
